Anyone who has worked in care knows how much can happen during a single shift. A change in someone’s mood, a missed meal, an unexpected appointment or a small concern about their wellbeing can all influence the support they need next. When one care worker finishes and another takes over, those details matter. A good handover helps make sure important information isn’t forgotten, outstanding tasks are understood and the person receiving care continues to feel supported.
To make that process easier, we’ve created a free care handover template for UK care assistants, support workers, home carers and nursing professionals. It’s simple to use, easy to print and designed around the information that matters during a shift change.

Why Is a Good Care Handover So Important?
Imagine arriving for an evening shift and discovering that the person you’re supporting hasn’t been feeling quite themselves. Perhaps they’ve eaten very little during the day, seemed unusually tired or needed additional help moving around their home.
If those observations haven’t been passed on, you could spend valuable time trying to understand what’s changed. Worse still, you might miss something that needs attention. A clear handover gives the incoming care worker a better understanding of the day so far. It provides continuity, helps staff recognise changes and reduces the likelihood of important information being overlooked. For someone receiving care, the difference can be reassuring. They shouldn’t have to explain the same concern repeatedly or wonder whether the next person knows about their needs and preferences.
Good communication is also recognised in professional guidance. The Care Quality Commission (CQC) explains that care records should be accurate, complete and up to date, while NICE highlights the importance of effective communication between health and social care professionals.

What’s Included in Our Free Care Handover Template?
We’ve kept the layout straightforward because a handover form should help you organise information, not create more unnecessary paperwork.
Shift Information
Record essential shift details so it’s clear when the handover took place and who was involved.
Health & Wellbeing
Highlight changes in a person’s physical health, emotional wellbeing, mobility or behaviour.
Medication Updates
Flag relevant medication concerns or updates that the incoming worker needs to check against approved records.
Outstanding Tasks
Make a note of anything that still needs completing or following up during the next shift.
The template also provides space for immediate safety concerns, next-shift priorities and handover confirmation, helping staff establish what has been communicated and what requires attention.
What Does a Good Care Handover Look Like?
The best handovers are clear, factual and focused on what the next care worker needs to know. They don’t need to be long, but they should contain enough information for someone unfamiliar with the previous shift to understand what happened.
For example, rather than writing “Mrs Smith wasn’t herself today”, a more useful entry might explain:
“Mrs Smith appeared more tired than usual during the afternoon and ate approximately half of her lunch. She said she felt less hungry than normal. Fluids were offered regularly, and intake was recorded in her daily care notes. The concern was reported to the senior carer at 2:30 pm. Please continue monitoring and follow the agreed care plan.”
Fictional example for illustration only.
Notice the difference. The second entry describes what was observed, what action was taken, who was informed and what needs to happen next.
That kind of detail helps colleagues make informed decisions without relying on assumptions. Where a concern requires urgent attention, staff should follow their organisation’s escalation procedures immediately rather than waiting for the next handover.

That’s why we’re developing a growing collection of free templates, checklists and guides for people working across the care sector. A Little Support Can Make a Big Difference
From newly qualified care assistants to experienced support workers and nursing professionals, everyone benefits from clear communication, good organisation and a supportive working environment. Our free resources are one small way of helping make that possible.
What’s the Difference Between a Care Handover and a Daily Care Log?
Although they work together, they serve slightly different purposes.
A daily care log provides a record of activities, observations and support throughout a shift. It might include meals, hydration, personal care, mobility and general wellbeing.
A care handover brings together the most important updates for the next person taking over. It highlights changes, concerns, outstanding tasks and anything requiring continued attention.
Think of the daily log as the record of the day and the handover as the conversation that helps the next shift begin with the right information. Both should complement your organisation’s existing care documentation procedures rather than replace them. For more detailed daily recording, you can also explore our
